By subscribing to or using the services of Dr. Pershang Ghaderi, the patient acknowledges and agrees that the fee for the medical services provided is not covered by RAMQ (Régie de l’assurance maladie du Québec). The patient understands that this is a private service and confirms they have been fully aware of the associated costs. By opting for these services, the patient does so of their own volition and accepts the financial responsibility of the charges incurred.
